hola a todas, estoy preocupada...
Os cuento: llevo en reposo por amenaza de parto prematuro desde la semana 25, ahora estoy de 31 justas.
Hace 2 semanas, o sea que estaba de 29, me pincharon dos dosis de corticoides para los pulmones del peque porque se me estaba acortando el cuello del útero y justo esta mañana he leído que esto hace q el bebe crezca más despacio, ¿es cierto?
Estoy preocupada porque la matrona me dijo que la niña debería nacer con algo más de 2 kilos para evitar incubadora (no me gustaría tener que separarme de él!)
Supongo q el reposo ayuda a que el bebé engorde, pero no se si es cierto lo de las corticoides que me inyectaron para los pulmones.
Encima el nene esta de nalgas y no se si ya se podra dar la vuelta, aunque me parece que de la cesárea no me libra nadie :(

Existen estudios que demuestran que el uso repetido de corticoides puede aumentar el riesgo de bajo peso al nacer pero estos estudios se refieren a dosis repetidas (dosis indiscriminadas dadas semanalmente). No es tu caso, Mamen. Con 2 dosis de corticoides no se ha demostrado este efecto que te comento. Te administraron corticoides porque vieron un riesgo claro de que el parto podía adelantarse y lo dieron con el objetivo de acelerar la madurez de tu bebé. Yo en tu lugar estaría muy tranquila.
Tal y como te han recomendado, sigue con tu vida tranquila y espero que llegues lo más lejos posible del embarazo

I have one query ..What is the protocol to give Inj steroids in your department ? I mean
1)Are the Inj Steroids repeated every week till baby reaches 36 wks ?because quiet a few hospitals do repeat the injections .
2) is it after the fibronectin test the injections are administered to the patient in true preterm labour ?

Welcome from India to this foro focused on prematurity. At our department, corticosteroids are only given in women with a high risk of preterm delivery. We don't repeat sistematically weekly doses ...just given if they are necessary. Only once (bethametasone 12 mg repeated at 24 h). Sometimes it is necessary to reapeat doses because patient is in labor and last one was > 1-2 weeks (but they are a few cases).

I hope you will share with us which is your current management of high risk. pregnancies

Trying to learn some spanish Mam . Thank you for the response Mam ,Well I was a bit confused about this Steroid injections because the Medical college where I have done my residency which is Indias most reputed hospital and research centre used a protocol to repeat Injections wkly till patient is in active labour but few hospitals where I am working dont use repeated weekly doses .Also my friends in UK use Fibronectin test so thats good because we know that patient is for sure in preterm labour .Few articles on Internet recommend and some dnt .
One more Patient I had few days back was a patient , Primi 36wks with Chicken pox ...lady was not vaccinated for chicken pox before and had full blown chicken pox . We started on anti viral drugs and thought of giving the baby Immunoglobulins immediately after delivery .This lady was not in labour .So we waited and watched and let the chicken pox subside .Well our worry was baby getting chicken pox and its harmful effects but when we delivered her after 2 wks she and baby were absolutely normal .Immunoglobulin was not available in India anywhere .
If the patient would have gone in preterm labour with the chicken pox what would be your proctocol ....Maam I am studying for my American registry of Sonography ob/gyn exams ,cleared my SPI exams ....though I have a 8 yrs experience of doing Sonography I am still tensed exams r exams ...Maam I am interested in reviewing any Fetal Abnormality ultrasounds so plz let me know if there is any such site ... Take care ..

If a women with chicken pox is in labor, guidelines recommendations is to AVOID patient will deliver IN THE FOLLOWING 4-5 days because it is very high the risk of congenital infection.. In this case, even in term gestations, tocolysis is allowed (the objective is to stop uterine contractions).
I am not sure if you want a recommendation of a book of fetal abnormalities or a website.
Please Clarify me

Thank you Maam .We did exactly the same ..There are quiet a few high risk cases we manage here ...I will surely update u if I encounter any interesting cases and if I need any advice ..My interest in Fetal Medicine has made me take up the American Registry for Diagnostic Medical Sonography exams because here in India Fetal Medicine is so much at its primitive stage .I wish I could do a Fellowship or a phd with your centre ..My dream is to be Fetal Medicine specialist .....Well Maam I have all the exam material from USA but i am looking for a website where I can see those rare Fetal Abnormalities detected on Ultrasound like Holoprosencephaly etc .....Kindly let me know ..
